accept HMSA, UHA and many other Hawaii-based insurance plans. I do not
accept new patients with Medicare, Quest, AlohaCare or Medicaid. Please bring an up-to-date insurance card to each visit. Remember to update my office when your insurance changes. My office will submit insurance claims for you. If you are aware that you have an annual deductible, please pay for your office visits at the time of service until your deductible is met. For questions, please call our Patient Accounts staff at 487-2277, press selection 3. If you experience financial hardship and need to make special arrangements for your payments, please speak with our Patient Accounts staff. Your continued health care is very important to me. |
